5 A Day in the Life- Acute Care Patient with a stroke Read the chart Speak with all relevant disciplines: RN, CNA, SLP, OT, RD, RT, Care Coordinator, physician. Get permission from the patient to see them for therapy Evaluate strength, sensation, mobility (rolling, sitting, standing, walking) Monitor vital signs Write my note in the chart.
