9 Evaluation Highlights 9 ACT leveraged hundreds of people, hours & expertise and over $2.5M in funding to: Successfully utilize a collective action structure and make progress around the 5 goals set forth by the ADWG Develop, test, and refine a Dementia Capable Communities Toolkit and process for encouraging voluntary communities/sectors to engage – 34 MN communities --national replication (DFAi) Develop evidence-based, culturally-responsive practice tools for health care systems and health professions training which are being implemented and embedded Influence state policy and legislative action – Economic model on counseling intervention published in Health Affairs; $1.5M in MBA grant funding for dementia care systems and community collaboratives
