2 MISSION STATEMENT Shelly Nuismer will read the mission statement which states: Community Mental Health of Ottawa County partners with people with mental illness and developmental disabilities and the broader community to improve lives and be a premier mental health agency in Michigan

3 WHO DOES THIS AGENCY SERVE? (Eligibility for Services) Community Mental Health serves those with mental illness as well as those with developmental disabilities

4 WHAT IS A DEVELOPMENTAL DISABILITY? When referring to an individual older than five years: It is attributable to a mental or physical impairment or a combination of mental & physical impairments It is manifested before the age of 22 It is likely to continue indefinitely Results in substantial functional limitations

5 PERSON CENTERED-PLANNING CMHOC develops Individual Plans of Service using person centered planning principles According to CMHOC: “The individual and their person- centered planning team will identify the individual’s dreams, desires and needs, and develop goals to address these throughout the processes of planning and provision of services and supports, honoring the persons preferences and choices as much as possible.” CMHOC also emphasizes the importance of identifying the individual’s role and responsibility in accomplishing the chosen goals.

6 SERVICES PROVIDED

7 SUPPORTS COORDINATION Supports coordinators help develop the person-centered plan and then check to make sure the services and supports continue to meet the individual’s needs

8 CLINICAL SUPPORT CMHOC has a team of professionals that are trained in nursing, psychology, occupational therapy, physical therapy and speech and language These professionals can assist with medical, behavioral, and skill building concern

9 COMMUNITY LIVING SUPPORTS Provide assistance, support, and/or training Focus on personal self-sufficiency, helping the individual become more independent and more integrated into their community These supports may include assisting, reminding, observing and/or training in: Meal preparation Laundry Shopping Money management Socialization and relationship building

10 SKILL BUILDING Provides knowledge, specialized skill development, and/or support to help individuals participate in school, volunteering, and/or in getting a job CMHOC provides this service directly at Community Based Services sites KANDU Pioneer Resources Skill Building Camp Indian Trails Camp MOKA Endeavers

11 SUPPORTED EMPLOYMENT Helps those who have the desire to work in the community CMHOC provides these services, and works with MRS and KANDU Services include: Job development Job coaching Long-term follow along services required to maintain employment

12 EXAMPLE OF A JOB PLACEMENT!

13 SERVICES FOR TRANSITION AGE STUDENTS Community Mental Health of Ottawa County will participate in school to community transition planning for eligible students with disabilities within two years of completing special education services (high school) Services Available during the two year transition period: Supports coordination Respite funding (funding is provided to the unpaid caregiver to purchase temporary care for the family member with a disability)
