4 13 NTSB Aviation Mishaps in February 2015 4 engine quit 2 unexplained (ham-fist or bucket-head) 1 iced up 3 gear collapse 2 pure bucket-head 1 unexplained (mechanical failure) Out of the 13 mishaps…4 were Fatal (8 fatalities total) Out of the 4 fatal mishaps…1 mishap pilot held an advanced aeronautical rating 1 Commercial (also, 1 had 2 foreign certified pilots…ratings not listed) NTSB Aviation Mishaps…Running Summary 2015 11 engine quit 7 unexplained (ham-fist or bucket-head) 5 unexplained (mechanical failure) 3 gear collapsed 2 pure bucket-head (Aviation Darwin Award category) 1 iced up

5 So….Who wins the Bucket-Head Award this month? 1. G4 Part 91 crew who ignored the Master Warning System 2. The Wheeler crew that flew a rough running airplane that they had to hot wire … 2 fatalities

6 Beginning October 1 st, 2015…the FAA is Proposing: 1. Flight Watch frequency 122.0 will be discontinued The services provided on 122.0 will be consolidated into routine inflight frequencies. The frequencies will be the same frequencies used to open/close flight plans or to receive updates on NOTAMS or TFRs. 2. Remote Airport Advisory (RAA) Service will be phased out at 19 Airports Airports do not meet the agency's criteria for receiving advisory service 3. Flight plans for civil aircraft will be filed using the ICAO format Will use the “International Flight Plan”

7 The FAA has a tutorial on how to fill this thing out on their website http://www.faa.gov/air_traffic/publication s/atpubs/fss/AppendixA.htm I recommend that we figure it out and build our own “Cliff’s Notes” tutorial. Also make a couple of “canned” flight plans.
