4. Progress Report Only Traditional Grantees report progress mid-project. 2 nd grant payment will be issued after report approved. Submit an updated budget – add a column with expenses to date. Report is due March 6, 2013. Receive instructions & reminder.

5. Final Report Both Traditional & Mini grantees submit Mini grantees can submit final report when grant activities are complete. Once final report is submitted and approved, the final payment will be disbursed. Submit updated budget – add a column. Traditional Grantees submit final reports by September 12, 2013., 4:30 pm. Receive instructions & reminder.

6. Behavior Change Evaluation For Traditional grantees only Submit survey results at the same time as the Final Report. Tool still in draft form, hope to have it ready soon. Will email as soon as it is complete, with instructions.
