1 Incorporating the principles of self-management into core Primary Care Key contacts/proposers: Pauline Sanders-Telfer, Lucy Hall, Claire Garbutt The idea Development and roll out of a training programme to general practice staff within CMDHB that will result in increased self - management conversations with patients with long term conditions. Who benefits and why Patient with long term conditions: Improved engagement with their general practice team, Greater understanding of their condition, Feel more in control of their health, Have improved health outcomes. Self-Management Support fit Being inspired: information, knowledge, role models and communication Any interaction with a healthcare professional is an opportunity for self-management support Activated patients and activated clinicians

2 Incorporating the principles of self-management into core Primary Care Key contacts/proposers: Pauline Sanders-Telfer, Lucy Hall, Claire Garbutt Aim: For every patient interaction within primary care to be an opportunity for the enhanced general practice team to provide tailored self management support by 30 June 2016 Measures: Number of self management intervention claimed through ARI budget

3 Huff and Puff – Spirometry on the Quit Bus Key contacts/proposers: Meg Goodman, Andrew Collingwood, Fiona Horwood The idea The first mobile spirometry screening service for people who smoke. People will be invited to be tested complete a symptom questionnaire have their results explained to them. Who benefits and why A mobile service brings the intervention to the population removing accessibility barriers. For individuals with early air flow obstruction, quitting smoking may prevent COPD developing If COPD is diagnosed, early interventions can alter disease trajectory and improve outcomes Self-Management Support fit Being inspired- information, knowledge, role models and communication Providing evidence of early lung disease may help provide added motivation and will power to quit smoking. Identifying early COPD means individuals have more control of their self management journey
