Initial Eligibility Determination for Students Exiting Part C of IDEA Part B of IDEA Stat Performance Plan Indicator B-12 Percent of children referred by Part C prior to age 3, who are found eligible for Part B, and who have an IEP developed and implemented by their third birthdays 12

Potentially Eligible for Part B Services Receiving Michigan Mandatory Special Education Services between the ages of birth and three. Child has an IFSP in place. Part C has referred the child to Part B special education 13

Process to Ensure the IEP is Developed and Implemented in a Timely Manner Part C refers the child to Part B between ages 27 months and 33 months of age Part B requests consent to evaluate Part B evaluates the child to determine eligibility for Part B of IDEA 14

Data Collection Districts must ensure that data is being submitted for students who exiting Part C and are potentially eligible for Part B services MSDS- both the Part C exit/transition component must be filled out and the initial IEP component in Part B 15
