The Problem  Though not new, long resident duty hours has come to the forefront as a serious concern for patients, their families, residents and fellows, and the medical community as a whole.  In addition to long hours, residents face: Shorter hospital stays – greater turnover means more paperwork. A sicker patient population. Financial cutbacks – widening the job description of the resident and fellow. Doing more in less time.

3. ACGME Duty Hour Standards  ACGME duty hour standards have been in effect since July 1, 2003  Since that time, many residency and fellowship programs have struggled to cope.  Residents are limited to a maximum of 80 duty hours per week, including in-house call, averaged over four weeks.  Residents must be given one day out of seven free from all responsibilities, averaged over four weeks.

4. ACGME Duty Hour Standards  Duty periods cannot last for more than 24 hours, although ~6 additional hours are permitted for the sake of continuity of care and educational activities.  Residents should be given at least 10 hours for rest and personal activities between daily duty periods and after in-house call.  In-house moonlighting counts toward the weekly limit. Program directors must ensure that moonlighting, both inside and outside, does not interfere with the resident’s achievement of the program’s educational goals and objectives.
