Version 0.4 The ‘Transforming Your Care’ Review In June 2011, the Minister for HSS&PS, Edwin Poots, MLA announced a Review of the Provision of Health and Social Care Services in NI. John Compton, Chief Executive of the HSCB was invited to lead the process in an ex officio capacity To provide a strong independent overview, the Minister appointed an Independent Panel: Prof Chris Ham, Chief Executive of the King’s Fund Prof Deidre Heenan, Provost and Dean of Academic Development at the Magee Campus, University of Ulster Dr Ian Rutter, General Practitioner Paul Simpson, retired senior civil servant Mark Ennis, Executive Chair of SEE Ireland. Report was presented to the Assembly in December 2012

Version 0.4 Vision and Scope of the Review Specifically the Terms of Reference were to: Undertake a strategic assessment across all aspects of HSC services; Undertake appropriate consultation and engagement on the way ahead; Make recommendations to the Minister on the future configuration and delivery of services; and Set out a specific implementation plan for the changes that need to be made in HSC.

Version 0.4 Real changes and improved outcomes Jean Better, more joined up assessment of her needs and more control over her own care budget Tom Better education to manage his long term condition, with care close to home when he needs it Gary More control over his daytime activities to enhance his health & wellbeing Joe Early intervention for mental health problems leading to earlier treatment Abby Quicker access to urgent care with specialist staff after a minor accident

Version 0.4 Following the review Draft Population Plans from each Local Commissioning Group area, and an overarching Strategic Implementation Plan were submitted by HSCB and PHA in June 2012 A period of Quality Assurance on the documents took place and updated draft plans submitted to the Minister on October 2012 Public consultation has been launched on Transforming Your Care: Vision to Action, and will be open until 15 January 2013 Subject to consultation, implementation will be over the next 3-5 years

Version 0.4 Strategic Implementation Plan themes Some of our proposals are not new but accelerate on- going processes of change Others signal significant change on how we organise HSC services Fundamental and managed change is needed to ensure we have safe, high quality and resilient services in future Strong focus on health and wellbeing and prevention targeted evidence based programmes to promote healthy lifestyle choice

Version 0.4 Strategic Implementation Plan themes Making it easier for people to access services, and quicker clinical decisions Provide an environment for innovation and new thinking with a strong emphasis on prevention and early intervention No hospital as an island – all acute hospitals will operate in a network Need to address fragility of our current services Some services will be more centralised than currently where there is evidence of better outcomes

Version 0.4 Moving forward There are no neutral decisions and we need to transform to ensure safe and resilient services into the future The model of care sets the individual at the centre, and gives them greater choice and control Shift of resources and re-investment in primary and community care – “home is hub” Building the capacity and capability of HSC organisations and their delivery partners to design, implement and sustain these changes will be key

Version 0.4 Consultation Launched Health Minister Edwin Poots MLA on 9th October 2012 Seeking views of everyone who use or provide services The consultation document Transforming Your Care: Vision to Action and draft Population Plans and Strategic Implementation Plan available to everyone on the TYC website or by contacting the HSCB Engagement includes: – Website – www.tycconsultation.hscni.net – Letter issued to over 500 stakeholders encouraging response – Leaflet sent to all households – 15-20 Public Meetings across Northern Ireland – Facebook & Twitter

Version 0.4 Consultation proposals – key highlights 17 Integrated Care Partnerships new ways of working for general practice, more integrated working across health and social care as part of multi-disciplinary teams Delivering care closer to home, reducing the need for avoidable visits to hospital, initially focussing on palliative care, older people and those with chronic conditions Support older people living independently and at home for longer Closure of 50% of statutory residential homes Completion of Bamford resettlement of long stay mental health and learning disability residents by March 2015 Develop 6 in-patient acute mental health facilities for those aged 18+

Version 0.4 Consultation proposals – key highlights Embed Family Support Hubs across Northern Ireland to focus on positive parenting and speech and language Ensure that mothers have access to midwife led care and the provision of ‘alongside’ midwife led units in major obstetric centres Acute hospitals services: – Regionally - £8m in Cath Lab services, £7m in orthopaedic services, development of protocols with ambulance service to ensure “right care, right place, right time” – Trustwide - Each Trust will continue to develop hospital networks and will have individual priorities for their own locality as outlined in the population plans, including option to change management arrangements for Causeway Hospital in Northern Trust Formal contractual arrangements with Republic of Ireland for population focussed and specialist services

Version 0.4 Consultation proposals – key highlights Change to staff profile, work being carried out in different areas or changed role as well as 3% overall reduction over next 3-5 years Significant role for IT in supporting these changes, with better sharing of information across HSC organisations Not about reduction in investment or spending but focus on making the best of the resources we currently have

Version 0.4 Implications for HR and staff CAPACITY : Workforce planning integral to planning and delivery of reforms: right people, right place THE ‘SHIFT LEFT’ : - shifting resources - supporting staff (incl VR / VER) - role of non-statutory sector CAPABILITY : All staff groups will feel a sense of changing role, and will get the training and development to build capability ENGAGEMENT : Staff at the forefront of leading transformation across all HSC and partner organisations Building capacity and capability across the HSC system is vital

Version 0.4 What happens next… The consultation is scheduled to end on the 15 th January 2013, at which point all responses will be reviewed Following analysis of the views from the consultation responses the draft Population Plans and Strategic Implementation Plan will be revised and finalised Subject to consultation, implementation will take place over 3-5 years Specific components of implementation will be subject to normal change processes, this will include further and more specific public consultation where appropriate
