Identify 3 interests or hobbies that you engage in often – write down your ideas on the line provided. STEP 1: PERSONAL INTEREST / SHAPE

1.Pick one of your interests that you will use for your “shape” 2.Separate your shape into 3 different areas – the areas will proportionately represent your own areas of health and wellness STEP 4: DESIGN YOUR SHAPE Your final product should not be larger than a normal sheet of paper – (8 ½ x 11). 1.You rated your areas of health – Write your ratings in the spacer provided. 2.You will use the proportions for your project – strongest health area will take up the most “area” of your shape. 3.Answer the question in this section about area. STEP 2: AREAS OF HEALTH

Write a paragraph and explain how the activity or interest you selected for your shape impacts all 3 areas of health and wellness. The shape I chose was a basketball because I spend a large portion of my free time playing. Basketball affects all three areas of my health and wellness. Basketball affects my physical health because I am exercising the entire time I play. In practice we not only run but lift weights and do other strength training exercises. Because I am physically tired I also tend to sleep better when I play basketball. Besides improving my physical health, basketball also has a positive influence on my mental and emotional health by lowering my stress. When I play basketball, I work out a lot of my frustrations from the day. ……… STEP 6: CONSTRUCTED RESPONSE
