2 Grant Information Program Goal: Providing support to the implementation of the Strategic Plan of Overcoming Propagation of HIV/AIDS epidemic in Tajikistan for the period of 2003-2005. Program Objectives: Creating opportunities for decentralization of preventive activities and expansion of intervention among vulnerable groups. Reducing vulnerability of intravenous drug users (IDUs), commercial sex workers (CSW) and youth. Ensuring rights of vulnerable groups through the strengthening of capacities of the government and civil society and the insurance of supply with necessary supplies. Building capacities to provide stronger leadership of the government on HIV/AIDS prevention. Better cross-sectoral coordination. Continued dialogue between the government and NGOs on central and local levels. Strengthening the capacities of the government officials on the central and local level, professionals in health, education and mass media, staff of law enforcement, NGOs as services providers to counteract HIV/AIDS through increased awareness and improved skills Activities HIV prevention among IDUs, CSWs and youth through improved access to Educational Information and Communication materials (EIC); Voluntary conscious testing (VTC); Improved means and awareness of individual protection (condoms as well as syringes and disinfectants for IDU); Treatment of sexually transmitted infections (STI); Improvement of laboratory facilities to ensure blood safety; Facilitate monitoring and evaluation of impact of prevention activities. Progress Summary Key Achievements The programme has significantly contributed to the implementation of the National Strategic Plan to prevent an HIV/AIDS epidemic in Tajikistan that was acknowledged by the Government and other key partners. This provided a good foundation for steady progress and future sustainability addressing new challenges and broadening and strengthening partnerships. Human rights concerns and related issues such as denial, stigma, fear of law enforcement agencies and negative attitude towards vulnerable groups have been addressed more concretely raising awareness of people about HIV/AIDS. The capacity of the national laboratories significantly improved leading to better blood supplies and the ability to accurately assess the HIV/AIDS situation in the country. Twelve trust points and eight trust centers were established and operate for HIV/AIDS prevention among IDUs and CSWs. Key Challenges Stigma remains a serious issue to both the physical and psychological well-being of people living with HIV/AIDS. Full support and assistance of the regional authorities is necessary for the sake of the successful programme implementation. However, it has been observed that there is not only a lack of understanding but also relative reluctance of regional authorities to realize importance of the programme objectives for the future of Tajikistan. It is important that Central Government authorities cast more accountability on to the regional authorities for Programme implementation. 3 Fact Sheet October 2004 TAJIKISTAN Grant Information Program Goal: This Programme is intended for a reduction of the social and personal burden of HIV/AIDS in the country. HIV/AIDS component aims to improve awareness of particularly vulnerable populations provide better access to prevention facilities and providing treatment of HIV/AIDS and STI.. Program Objectives: Prevention of HIV transmission among migrants and their families in urban and rural areas and among street kids in cities; Prevention of HIV transmission among prisoners nationwide; Scaling up the access of people living with HIV/AIDS to effective treatment Activities: Scaling up information, education, communication and condom promotion among labour migrant and street children Provision migrants, their families and street kids with better access to voluntary counselling and testing Provision of target groups with HIV and STI prevention and health care facilities Scaling up information, education and communication in prisons aimed at increasing awareness about HIV/AIDS and STI among prisoners Provision prisoners with HIV preventive equipment including condoms, lubricants and disinfectant Syringes exchange in the pilot prison aimed at gradual expansion of the program in other prisons of the country. Provision prisoners with better access to services for diagnosis and treatment of sexually transmitted diseases Provision the access of people with advance HIV infection to antiretroviral therapy Treatment of opportunistic infections PLWHA Provision of the access of HIV infected pregnant women to complete course of antiretroviral prophylaxis to reduce the risk if MTCT Improving diagnostics of HIV, opportunistic infections and disease monitoring, including development of laboratory facilities for determination of CD4 level and viral load. Building national capacities on provision of treatment, care and support to PLWHA and HIV and STI prevention services to vulnerable groups through provision of training to health care professionals and NGO/CBO volunteers (peer educators). Programmatic Details Grant Number:TJK-404-G03-H Grant Title:Reducing burden of HIV/AIDS in Tajikistan (Round 4) Principal Recipient:United Nations Development Programme in Tajikistan Budget:Two Year Budget: USD 2,508,720.00 Program Start Date:1 January 2005 Role of UNDP in addressing HIV/AIDS in Tajikistan An important objective of the HIV/AIDS-related activities of the United Nations Development Programme (UNDP) in Tajikistan is to support the country to strengthen and expand its capacity to respond to the development implications of the epidemic. Activities are focused on identifying effective and sustainable policy and programme responses surrounding the epidemic’s social and economic implications. The aim is to strengthen capacity for an effective response within government and civil society as well within UNDP itself. UNDP Country Office in Tajikistan could serve its role as a coordinating mechanism in order to mitigate the following challenges: Lack of coordination of activities between the government and international agencies in fighting HIV/AIDS; Absence of unified national system for monitoring and evaluation; Limited capacity of the government to provide efficient surveillance of and effective response to HIV/AIDS spread in the country; Lack of sense of ownership among the national organization as well as civil society with regards to programs implemented to combat HIV/AIDS in the country. In partnership with many organizations, UNDP could emphasize support to initiatives which catalyze community and national mobilization; create a supportive ethical, legal and human rights framework; are gender sensitive; empower people to take charge of their own well- being, drawing on local resources and building on local knowledge and values; and foster an enabling political, economic and social environment. Subject to effective and efficient coordination, the UNDP in Tajikistan could provide a catalyzing mechanism for fostering intercountry collaboration and helping national institutions and networks to strengthen their capacity to respond effectively to the epidemic.
