1 How to Respond to Requested Changes and Provisos

2 A) Responding to:  Changes Required by Department Review  Changes Required by C&W Peer Review; or  Changes Required by REBA

3 Identify the application requiring changesClick on the Name of the study for which you would like to make changes to

4 Right-click and open in a new window to view the reviewer’s detailed comments and instructions

5 Right-click and open in a new window to view the REBA’s detailed comments and instructions

6 View changes requested by the C&W Hospital during their initial review

7 Click to make the changes requested of your application

8 Page 1 of the Application Use the Jump to: tool to navigate the pages of the application

9 Use the Jump to: tool to navigate the pages of the application Click “Save” between pages and before you “Exit” the application Make the requested changes in the appropriate sections of the application

10 Click to Submit the revised application back to the reviewer

11 You may want to copy and paste the list of requested changes and your responses in your comments to the reviewer Click “Ok” to send your application back

12 Notice the state of your application has returned to the reviewer

13 B) Responding to Provisos

14 Identify the application that has provisos issued by the REBClick on the Name of the study for which you would like to address the provisos

15 Right-click and open in a new window to view the REBA’s detailed comments and instructions

16 Right-click and open in a new window to view the REBA’s detailed comments and instructions

17 View changes requested by the C&W Hospital during their initial review

18 Click on the Provisos tab to view the list of provisos issued by the REB

19 List of provisos – Click on the title for more details Click to make the changes requested of your application

20 Use the Jump to: tool to navigate the pages of the application Make the requested changes in the appropriate sections of the application Click “Save” between pages and before you “Exit” the application

21 You may want to cut and paste the proviso items into a MS Word document and address each with a response or explanation. As proviso items are usually quite lengthy, a concise and detailed proviso response can further expedite the review process

22 Click to submit your responses to the provisos issued by the REB

23 Enter any comments you would like to communicate to the REBA regarding your provisos Add your proviso response as an attachment Click “OK” to submit the provisos back

24 Notice the state of your application has gone to “REBA Screening Provisos”
