It covers everything someone with asthma needs to know to control his/her asthma. It offers an interactive and flexible format to suit your needs. Materials are available in English and Spanish – allowing you to reach those in your population that are Spanish-speaking. When a person truly understands their asthma, uses today’s medicines as directed, and follows strategies to decrease lung irritation—they can enjoy a wide variety of activities. - Mark T. 8 www.lungusa.org Why Breathe Well, Live Well ® ? It helps reinforce your hospital as a center of excellence in pulmonary rehab; and extends your quality of care. It aids in building rapport with your employees/members/ population. It helps lower overall absenteeism and presenteeism that helps keep productivity levels high. It reduces asthma-related illness and disabilities which reduce the healthcare costs of those with asthma.

9 www.lungusa.org Program Development Developed with funding from the Centers for Disease Control (CDC) Based on a validated program developed by the University of Alabama at Birmingham Lung Health Center (UAB)

10 www.lungusa.org Program Delivery The course is delivered by an American Lung Association-trained facilitator to a group of up to 10 individuals. Group clinic allows your population/employees/ members the opportunity to benefit from group dynamics and learn from their peers.

11 www.lungusa.org Program Delivery It’s flexible! – There are three standard options to meet the needs of the organization/facilitator and participants. –Orientation module – The program begins with a 15- minute phone call explaining to participants what they can expect from the program and what to bring to the workshop. –Workshop module – Includes an interactive 4-5 hour module featuring lecture, group discussion and skills practice to support asthma management. The number of sessions in which this module is delivered is flexible; you can offer it in 1 intensive session or spread out over 2 or 3 sessions. –Reinforcement module – The program concludes with a 15-minute phone call to participants for reinforcement and follow-up.

12 www.lungusa.org Program Content The BWLW program teaches: Understanding asthma and how it affects breathing Getting help from healthcare providers Learning about asthma-control medicines Learning how asthma relates to other healthcare conditions Finding asthma triggers, tracking symptoms, and creating an asthma action plan Forming good health habits “Having my asthma under control improves the quality of my life. It means that I can work in the yard, play outside and most important, read a bedtime story to my daughter without having to catch my breath or gasp for air.” -Mother with asthma
