Once the requisition is validated, you cannot modify it anymore. You can communicate with the Actilab Customer Service department if there are modifications to be made to the requisition after has been validated. Press “Cancel” to abort the current operation and make a new requisition.

7. When this window opens, the print dialog will pop up shortly thereafter. Follow instructions to print. Requisition – New Printing Copy After clicking the “Validate” button in step 3 this wimdow will appear. Wait until you have been prompted to print before cicking the « Ok » button

9. Requisition – Results Step1 Select what requisitions you would like to show. You have the following choices: “Your Name” : Shows the requisitions you have made “For the Company” : Shows requisitions made by you and any other employee from your company To narrow down your search options, you can choose from the Following options: “List all requisitions”: This will list all requisitions “By Product”: To search by product name “By Lot”: To search by lot number “By Requisition”: To search by requisition number

Requisition – Results Step2 NOTE: You may not see this step By trying access this step you can end up in: Results View page Cause: You have chosen “List all requisition” under “Search for” option in step1 You receive a message telling you that you have no requisitions Cause: You or your company haven’t yet made any online requisitions. According to your choice in step one, you will have a list to choose from, either: Requisition number ( this example) Lot Number Product Name

11. Requisition – Results View An empty field or and hyphen (-) means that the requisition or test is not yet completed

13. Requisition – Payment Step 1 Select requisition that you would like to pay After selecting desired requisition to be paid click “Next Step >>”. Or “Cancel” to go back to your account

Requisition – Payment Step 2 The total amount of selected requisitions with taxes (where applicable) appears here Fill up the form After Clicking “Validate” you may see two possible responses: “Your transaction is accepted” NOTE: NO MONEY TRANSFER HAS BEEN MADE YET! Your information seems to be valid and the actual transaction will be made soon. “Your transaction is declined” You have possibly made a mistake while filling out the form. Verify the information in “Payment” section one more time.

Quotation - Add Step 1 Select product for which you want add a test, a method and a specification NOTE: if you have come from Requisition Step 2 the product you have chosen for the new requisition will be selected by default. Product/Method/Test What you want is not in the list ??? !!! Fill in the fields for your new product/method/test Select Method, Test and type in the specification

24. Quotation - Add Step 2 The product’s identification and version information appear here if the product is already defined in our database. If not, there will be empty fields where you can add the product identification and version information, if you wish to do so. Hit “<< Prev Step” to go back to Quotation Step 1. Hit “Validate” to submit quotation. (Make sure that all information is accurate beforehand, as you will not be able to modify it after it is validated)
