2 CONFIDENTIAL MATERIAL | © 2014 BaseHealth Overview In this guide, you will learn how to find and enroll new members & physicians, view your enterprise group’s health performance, and make changes to your profile and settings. To begin this training, we will take a look at the home page of Genophen’s Enterprise Group user interface.

Welcome to the EG Admin Training Guide! In this walkthrough, you will learn how to navigate the EG Admin user interface This is the homepage. From here, you will be able to do add physicians to your enterprise group. To do this, click “Add” to the far right of the physician’s name. To introduce your own physician to the Enterprise Group, click “Introduce Your Own Physician”

Here, you will be able to introduce new physicians to the enterprise group. To send an invitation, simply input their first & last name, email, and preferred language, and click “Invite” at the bottom of the page. Next, you will learn how to enroll new members in your enterprise group by clicking “Enroll Member”

This is the Enroll Member section of the Enterprise Group user interface. Here, there are two ways to enroll new members to your Enterprise Group: Members can be enrolled individually, by inputting their first & last names, email, and preferred language. Alternatively, members can be enrolled in a batch by using “Batch Upload” which invites members en masse, using an uploaded.csv or.xlsx file. To enroll the member, click “Enroll” Next, click “Performance”

From Performance, you can view the health performance of the members of your enterprise group. The percentage of patients with significant risk for the diseases is shown on the right. If you would like to view other metrics, click “Other Metrics”

Within Other Metrics, you can view the Behavioral Risk, Lab Work Risk, & Biometrics Risk, Take a look at the “Filter by Physician” option to the left.

By selecting a physician from the enterprise group, you can view the performance of that physician’s members, instead of that of the entire enterprise group.

Next, click “Members”

This is the Members List. Here you can review the member information of your enterprise group. There are three tabs: Invited, Awaiting Consultation, and Consultation Complete. From the Invited tab, you may view members that have been sent invitations to the enterprise group, but who have yet to create their account. In the Invited tab, you have the ability to send another invitation by clicking “Resent Enrollment Mail” Next, click “Awaiting Consultation”

This is the Awaiting Consultation list of Members in your Enterprise Group. Here you can view members information, or lock their accounts. To lock a member, click “Lock” on the right-hand side. Locking the account will prevent the member from making use of the platform. Members can be unlocked by clicking “Unlock”, which will appear in place of “Lock” for members with locked accounts. Next, click “Consultation Complete” The Awaiting Consultation list, is the list of members in the enterprise group who have been invited, created their account, but have not yet had their results consultation with their Enterprise Group physician.

This is the Consultation Complete section of the Members List. Here you are able to view members who have completed their results consultation with their Enterprise Group physician. Members can have their accounts locked by clicking “Lock” to the right of their names. To return to the home page, click “genophen” in the upper left-hand corner.

If you would ever like to make adjustments to your account information or settings, click your name in the top-right corner of the user interface. Welcome back to the home page!

After selecting your name, a dropdown menu will appear. To continue, click “My Account” You may additionally click the “?” icon in the top-right corner to the left of your name to access the Genophen’s help section. Alternatively, you may click “Support” in the dropdown menu to access our help section.

This is the My Account section of the Genophen user interface. From My Account, you can view and/or make changes to your Personal Contact, Enterprise Group, or Settings. From Personal Contact, you can make changes to your first & last name, preferred language, primary phone number, or email. To confirm any of these changes, click “Submit” Next, click Enterprise Group

This is the Enterprise Group section of the My Account portion of Genophen’s user interface. From Enterprise Group, you can view and/or make changes to your company name, company logo, company address, or office phone number. To confirm any of these changes, click “Submit” at the bottom of the page. Next, click “Settings”

This is the “Settings” section of the My Account portion of the Genophen user interface. There are four sections, two of which can be viewed at the top. You must scroll down to view the next two sections, which will be covered in the following slide. At the top of the page, you can make changes to your personal authentication preferences, notification preferences, or password. To confirm any of these changes, be sure to click “Save”

You can scroll down on the “Settings” page to find two more sections: Pin, and Security Questions. Here you can change your account Pin number, or make changes to the security questions used to authenticate the account. Be sure to click “Save” after making any changes.
