Outreach Team Presentation By Shannon Skerratt and Denise Chambers.

The Outreach Team Outreach is a team of 5 nurses, with access to a small pool of bank nurses. We are committed to providing a high standard of palliative care in the community (home and residential homes). For patients and their families with cancer or other life threatening illnesses such as COPD, MND, and heart failure.

We are part of St Johns’ Hospice in Lancaster. The Outreach Team We work from 0830am-1630pm seven days a week, including bank holidays and weekends. Our catchment area covers South Lakes, (such as Ambleside and Windermere) down to Catterall and across to Yorkshire Border (such as Ingleton). However, we will visit any patient known to St Johns’ Hospice.

The Outreach Team Outreach’s’ remit is to support people who choose to be cared for at home by working closely with mainstream services such as: district nurses, GPs, Macmillan nurses, care agencies, and the patients’ carers. Our aim is for palliative care patients to have equity of access to services and resources irrespective of where they live. To enable them to be cared for and die in the place of their choice and offer support to those who care for and matter to them.

The Outreach Team Emotional and psychological support. Involvement in Hospice MDT discharge planning meetings. We are a specialist Palliative Care resource and work in support of and compliment the primary health care team and Marie Curie cancer care. We are able to provide: End of Life Care including administration of 4 core drugs, reloading syringe drivers. Personal care (short term only) whilst waiting commencement of/or in addition to care package. Facilitate hospice and hospital discharge for patients who wish to die at home. Prevent crisis admission e.g chest infection; UTI. Sometimes when situations improve Outreach service may then no longer be required, but can be resumed. Respite

Outreach team service has access to the following professionals at St John’s Hospice: The Outreach Team Lymphoedema service Bereavement services Chaplain Occupational therapist Physiotherapy team Hospice day therapy Hospice doctors Slynedales

Referral to the Outreach service may be made 08:30am-16:30 7 days a week with an initial telephone call followed by a faxed referral. Referrals accepted from all members of the MDT and also patients/families may self refer. We will accept referrals for those aged 18years and above. The Outreach Team

Improving the quality of life of patients and families who face life-threatening illness, by providing pain and symptom relief, spiritual and psychosocial support to from diagnosis to the end of life and bereavement.
