1 TRIBUNAL WAIVER Form(s) INSTRUCTIONS in the use of The new Printable Version and The new automated Version

2 Printable Version (to hand enter data) Download (save) form to your hard drive and print or … print directly from the document library. Do not attempt to key information into this form … formatting will not be correct. Use the automated form if the typing or keying of the fields is necessary. Hand enter all information to this printed version. If your printer adversely adjusts the formatting, contact the Department of Administrative Services. Developed in an older version of WORD which should maintain compatibility with newer versions of the software. If attachments are used, be sure to reference the attachments somewhere on the form by hand.

4 Automated Version Download (save) form to your hard drive and fill in the fields. Save your work often to avoid loss. The signature area (including dates) is designed for hand written entry and will not allow keying in this area. This form is designed to flow to multiple pages when necessary. Select the student’s enroll school from either the elementary or middle/high box. Select the student’s ethnicity as it appears in the student information system. Select the student’s gender as it appears in the student information system. Select the student’s grade as it appears in the student information system. Select the student’s age. Key in the student’s ID (not social security #) To use the fields (unlimited in size), place your cursor over the line (field) and key the student’s Name (as it appears in the student information system). Repeat this step with all fields. There is a small field between the Special Ed reference block and the signature block that allows line spacing to be added in order to move the signature block down for printing. Use this field if needed only after the other fields are complete.
