6 | 6 1. Earn the Gold Reward by scoring 90 or more points. 2. Earn the Silver Reward for scoring 80 or more points. 3. If you do not score in one of these two categories, you will not earn a reward for 2014. Find your Rewards score in the chart below to see what reward you qualify for in 2014. The scoring is very simple and specific: Society Insurance Rewards Program Gold Reward Score 90 or higher Silver Reward Score 80 or higher No Reward Score 79 or below

7 | REWARD PROGRAM Gold Reward (Score 90 or higher) Employee $20 per pay check for employee on the health plan that achieves the gold podium $125 one time payout, for employees that are not part of our health plan and achieve the gold podium Spouse or Same Sex Domestic Partner $10 per pay check for spouse on the health plan that achieves the gold podium Spouses not part of our health plan are not eligible for a reward 7

8 | REWARD PROGRAM CONTINUED… Silver Reward (Score 80 or higher) Employee $10 per pay check for employee on the health plan that achieves the silver podium $75 one time payout, for employees that are not part of our health plan and achieve the silver podium Spouse $5 per pay check for spouse on the health plan that achieves the silver podium Spouses not part of our health plan are not eligible for a reward 8

9 | EARN BACK POINTS PROGRAM Complete health screening Complete Know and Go Questionnaire Meet with health coach and achieve scorecard healthy ranges or goals indicated on the points earning criteria document. Health coach onsite once per month Utilization report (8 times per month), proof of membership, proof of payment, HRA participation 9

10 | HEALTH CLUB POLICY Full and part time employees receive a free membership to Club Olympia In addition reimbursement program for employee themselves and a family member Up to $30 per month for employee Up to $50 per month for employee and family member 10
