8 2. Student Growth Objectives (SGO)  List of assessment/s used to measure SGO  SGO Goals  Deadlines established to meet SGO goals  Supplemental Artifacts:  Copies of assessment tools

9 3. Evaluations & PIP  Evaluation Reports for the current school year  A copy of the previous year’s PIP for everyone not new to the district  Supplemental Artifacts:  Any informal observations recorded
