2 8/16/2015 2 Introduction All injuries/illnesses must be reported to Environmental Health and Safety using the BGSU Injury and Illness and Follow-Up Report. You will learn:  how to report an injury/illness;  BGSU’s procedures for reporting safety hazards;  what your duties as an employee are and;  what the Universities duties as an employer are.

6 8/16/2015 6 Definitions Injury: any wound or damage to the body resulting from an event in the work environment (i.e. cut, puncture, laceration, abrasion, fracture, bruise, contusion, chipped tooth, amputation, insect bite, electrocution, or a thermal, chemical, electrical or radiation burn. Sprains and strains to muscles, joints, and connective tissues are classified as injuries when they result from a slip, trip, fall or other similar accidents.)

7 8/16/2015 7 Definitions cont… Illness: any abnormal condition caused by exposure to environmental factors, (skin diseases or disorders, respiratory conditions, poisoning, temperature related illnesses, effects of radiation, bloodborne pathogens, anthrax, etc.)

8 8/16/2015 8 Definitions cont… Work related injury or illness: an injury or illness is considered work related if an event or exposure in the work environment caused or contributed to the condition or significantly aggravated a preexisting condition. It is presumed for injuries and illnesses resulting from events or exposures occurring in the workplace, unless an exception specifically applies. This includes the establishment and other locations where one or more employees are working or are present as a condition of employment.

9 8/16/2015 9 Definitions cont… Employee: one who is employed in the business of his or her employer affecting commerce (anyone receiving a paycheck from BGSU). Days away from work: the number of calendar days, beyond the day of injury or onset of illness, that the employee was away from work as a result of a recordable injury or illness.

10 8/16/2015 10 Definitions cont… Restricted workdays: the number of calendar days, beyond the day of injury or onset of illness, on which the employee remained at work and the employer or health care professional kept or recommended keeping the employee from doing the routine functions of his or her job or from working the full work day that the employee would have been scheduled to work before the recordable injury or illness occurred.

11 8/16/2015 11 Definitions cont… Routine Function: those work activities the employee regularly performs at least once a week.

12 8/16/2015 12 Recordkeeping Regulations The PERRP adopted the recordkeeping guidelines for occupational injuries and illnesses established by OSHA and effective in April, 1986. The federal standard changed in January, 2002. BGSU converted to the new system effective January 1, 2004.

13 8/16/2015 13 Recordkeeping Regulations cont… The recordkeeping regulations do not apply to students, visitors, and guests. However, we ask that all injuries and illnesses be reported via the BGSU Injury and Illness Report.

14 8/16/2015 14 Recordkeeping Regulations cont… OSHA 301 Form or equivalent: injury/illness incident report –Initial form that must be completed. –Is used to help employers and OSHA develop a picture of the extent and severity of work- related incidents.

15 8/16/2015 15 Recordkeeping Regulations cont… OSHA 300 Log: log of work related injuries and illnesses –Used to classify work-related injuries and illnesses and to note the severity of each case, what happened and how it happened.

16 8/16/2015 16 Recordkeeping Regulations cont… OSHA 300 Log: log of work-related injuries and illnesses –Work-related injuries and illnesses that result in: Death Loss of consciousness Days away from work Restricted work activity or job transfer Medical treatment beyond first aid

17 8/16/2015 17 Recordkeeping Regulations cont… OSHA 300 Log: log of work related injuries and illnesses –Additional Criteria Significant injury or illness that is diagnosed by a physician Cancer, chronic irreversible disease, fractured or cracked bones, or punctured eardrum Any needle stick injury or cut from a sharp object that is contaminated with another person’s blood or other potentially infectious material Any case requiring an employee to be medically removed under the requirements of an OSHA standard Tuberculosis infection as evidenced by a physician or other licensed health care professional after exposure to a known case of active TB.

18 8/16/2015 18 Recordkeeping Regulations cont… OSHA 300A Form: summary of work related injuries and illnesses –Shows the totals from the OSHA 300 log for the year in each category –Is posted at the end of the year in Environmental Health and Safety, Human Resources, McFall Center, Dining Services, Facilities Services, and the Bowen- Thompson Student Union. The revised BGSU Injury/Illness Report form requests information that satisfies requirements of the OSHA 300 form and the OSHA 301 Log.

21 8/16/2015 21 Frequently Asked Questions? Do I count the day on which the injury or onset of illness occurred when counting days away from work or restricted work days? NO Do I count days away from work when the health care professional recommends the employee stay home, but they come to work anyway? YES

22 8/16/2015 22 Frequently Asked Questions? Do I count days away from work when the health care professional recommends the employee return to work, but they stay home anyway? NO How do I count weekends, holidays, or other days the employee would not have worked anyway? You must count calendar days, including weekends, holidays, or other days off, the employee was unable to work as a result of the injury or illness whether they were scheduled to work or not.

23 8/16/2015 23 Accident Investigation BGSU has an Accident Investigation Report that may be used to find the main causes of injuries, illnesses and near misses, and to take corrective action. The Accident Investigation for Supervisors training class explores the details of an accident investigation.

24 8/16/2015 24 BGSU Procedures for Reporting Safety Hazards TAB 3 Employees are encouraged to correct unsafe conditions as follows:  Report them to the next administrative level.  If this notification does not correct the hazard, or if the hazard is not in the employee’s department, call Environmental Health and Safety at 372-2171.  Examples of reportable conditions: fire hazards, trip/fall hazards, unsafe equipment, improper use of equipment, damaged asbestos, no personal protective equipment, or improper chemical use or storage. Call Environmental Health and Safety or Human Resources (372-2259) with questions about this poster or about reporting procedures.
