3. **********to save a copy and begin working with form************ Choose File>Save As>your folder name Change name to Mat-100 standard file name convention name Click Save **NOTE** Please use link on website each time you create a new Mat-100 as new data is being constantly added.

4. Fill out Mat-100 - Sample ID (example format T0033-0103B001) - If sample is in English measure, choose “English List” left drop down in sample unit (see above), otherwise choose Metric and select appropriate unit of measure - Phone number to be entered in 8605943000 format - Contract number entered in 033103 format (hypen will be generated) - All fields with * are required

5. After completion of document, click File>Save To forward document to District in D.O.T. Click File > Attach to Email Mat-100 will be attached to email and can be sent. If email does not come up, open your email, format an email to send and attach document from where it was saved

6. Type in where Mat-100 should be sent to, using the email addresses at the bottom of the Mat-100 Send email

7. DISTRICT USERS: Open pdf file attached in email. Adobe will open with Mat-100 displayed Save Mat-100 on local computer Choose File>Save As>your folder name Review, make necessary changes on Mat-100. Choose File>Save

8. To send Mat-100 in format for Site Manager Click “Submit to SM” at bottom of form (email will pop up with file in subject line) User brought to email with document name in attachment Right click on attachment Choose “Save As” Save in specified directory for Site Manager Save in specified directory for Site Manager (be sure not to overwrite existing file names) “Select Email Client” box appears Choose “Desktop Email Application” and click “OK”
