2 Foundation Simulation Workshop Our Aims for Today: To provide an overview of the simulation experiences provided for Foundation doctors across KSS and London To consider the obstacles to growth and opportunities for enhancing Foundation simulation across KSS and London To further develop the effectiveness of simulation for Foundation doctors

3 Simulation and Foundation UKFPO Foundation Curriculum 2007 – Education and Assessment Simulation training should be fully exploited. CMO Annual Report 2008 section- ‘Safer Medical Practice: Machines, Manikins and Polo Mints’ – highlighted the need to formally integrate simulation and human-factors training into medical education and practice.

4 Simulation and Foundation STFS Website Simulation Training All Foundation Doctors based at London or KSS Deanery Trusts have access to simulation training FY1 sign-off requirement FY2 within KSS limited access

5 STeLI Simulation Technology-enhanced Learning Initiative Excellence in Education strategy London Deanery and NHS London e-Learning and Simulation promote high quality service delivery development of professional capabilities crisis resource management effective communications team work and leadership skills patient safe environment

6 STeLI Investment & Development 32 London Acute Hospital Trusts – Equipment Medium fidelity simulator – Facilities Simulation suites Faculty Development Centres (Bart’s St Georges UCL RFH Chelsea) – ‘training the trainers’ course – 1700 facilitators

7 Education, Research and Conferences MA Education and Technology in Clinical Practice STeLI Stakeholders' Forum Jan 2010 – New Patient Safety Human Factors Training Resources for Trusts – The future direction of simulation in London's healthcare community 3rd Annual Simulation Conference March 2010 – 'Innovation in Education - the engine for change.

8 STeLI and STFS FY1 & FY2 in London based STFS Trusts have access to simulation training days Foundation Faculty and Simulation Faculty within these Trusts have funded places on Faculty Development course run by STeLI Infrastructure investment STFS keen to develop a more integrated simulation strategy across London and KSS Deaneries

9 KSS Deanery Simulation KSS Overview Rapid growth in numbers: 2007-2008: 120 2008-2009: 300 2009-2010: 491 * *NB Added to sign off for ALL trainees in 2009-2010, so provision allocated for 520 places, as safety net needed

10 KSS Deanery Simulation KSS Overview 7 of our 11 NHS Trusts have now developed local provision for FY1 8 potential hospitals have yet to develop local simulation for FY1 Two NHS Trusts providing ‘partnership’ simulation for FY1s: Brighton and Royal Surrey County Only 3 KSS NHS Trusts now using London specialist centre provision

11 KSS Deanery Simulation KSS Overview Targets: To expand simulation provision into FY2 To ensure all NHS Trusts have at least one clinical skills facility To expand simulation provision within core and specialty training

12 KSS Deanery Simulation Vision: All Foundation Year 1 trainees will receive at least one experience of high-fidelity simulation within their placement at their local education provider NHS Trust. This will be delivered by local faculty released formally from service delivery and the experience quality-assured by the Deanery with a visiting team drawn from the community of practice across the KSS region operationalising national and KSS quality standards.

13 KSS Deanery Simulation Current initiatives: Big capital investment (£756,895) investment in 2009 bidding round Recent initiation of Quality Support and Development visits to NHS Trusts Simulation steering group now established (five colleagues) Simulation Special Interest Group (SIG) being developed Appointment of speciality champions to further develop simulation

14 STFS Simulation Provision Overcoming Barriers Please discuss the following points and respond to the area allocated to your group: 1.Obstacles to growing/improving simulation in NHS Trusts (all groups) 2. Opportunities for enhancing simulation experience Group a. Introducing FY1s to the simulation experience b. Developing effective scenarios for Foundation doctors c. Developing active and participatory watching d. Moving from ‘debriefing’ to ‘professional conversations’
