2 USC and Eric Cohen Student Health Center The ECSHC is funded by the student health fee – This is a mandatory fee charged to all students that cannot be waived $273 for the Fall Semester and $272 for the Spring Semester – Students can use ECSHC services regardless of insurance carrier. The ECSHC serves the students of the USC Health Science Campus as a healthcare resource by: – Processing incoming students’ health clearance paperwork – Providing medical and counseling care without charging visit fees – Providing free in-house laboratory tests such as pregnancy tests, rapid strep tests, etc. – Providing free needlestick/blood borne pathogen care received at the ECSHC – Providing free yearly TB skin tests – Filling out and signing applications for internships and clinical rotations Eric Cohen Student Health Center of USC Call 24/7: (323) 442-5631 | ecohenshc.usc.edu

If you have your own private insurance & want … – To see a primary care physician = $0.00 – To see a specialist = $0.00 – To see a counselor = $0.00 – To receive blood work, vaccines, or procedures = Prices will vary. Must pay all costs out of pocket. You can submit the receipt to private insurance for possible reimbursement (not guaranteed). The ECSHC does not accept payments from private insurance companies If you have the USC Aetna Student Health Insurance & want… – To see a primary care physician= $0.00 – To see a specialist = $0.00 – To see a counselor = $0.00 – To receive blood work, vaccines, or procedures = Prices will vary. Costs of preventative tests are sent to Aetna. Other tests must be paid out of pocket. The ECSHC can submit your receipts to Aetna to be added to your deductible. 10 USC and Student Health Insurance Every USC student is required to have health insurance – A health insurance fee is automatically charged to your account but it can be waived. $297.00 for the Summer $610.00 for the Fall $1,102.00 for the Spring – If you have your own private insurance, you can apply to waive out of Aetna Student Health Plan. The fees are automatically posted on your account and will not be removed unless your private insurance is approved. SUMMER WAIVER DEADLINE: July 03, 2015 FALL WAVIER DEADLINE: September 11, 2014 – If you want to keep the Aetna Student Health Insurance, you do need to do anything. – Please visit our insurance coordinator for more information! 12 Student Health Insurance Quick Facts Health Insurance coverage is from August to August – OT Students start June 15, 2015 and go until August 16, 2015. – You will have to enroll in the insurance every Fall semester You have a $450 deductible per year – You’re deductible starts June 15, 2015 – Reset on August 17, 2015. Dental insurance applications are only available in the Fall – Delta Dental is the USC sponsored Dental insurance – You can start enrolling now. – DEADLINE: September 11, 2015 – You have to fill out a form available online and at the ECSHC and submit it to Sylvia Vision care comes with your health insurance purchase. – Eyemed is the USC sponsored Vision Care – It’s a two for one deal (When you buy aetna student health, you get vision care with eyemed) – It comes automatically with Aetna Student Health The Eric Cohen Student Health Center is your primary care – You must receive a referral from our physicians anytime between August 18, 2014 and May 15, 2015 Eric Cohen Student Health Center of USC Call 24/7: (323) 442-5631 | ecohenshc.usc.edu

13 USC and Student Health Insurance Summary of Plan Changes USC Student Health Insurance Plan Changes for 2015-2016 – The $50 Annual pharmacy deductible will be eliminated, student will continue to pay a copay for generic and brand drugs. The generic copay will not change. However, the brand copay will change from 10% with $25 minimum to 20% with $30 minimum. – The emergency room copay will be increased from $150 to $200. – A $100 copay for ambulatory outpatient surgeries will bed added.
