6. Access to Student Records Parents do not have access to student health records without the students written permission Students may request a copy of their medical records for a visit, etc. by signing a release for that specific visit ALL students admitted in or after Fall 2009 must have health insurance. 7. Immunization Forms Immunization forms MUST be submitted to the Health Center during Orientation Students will be blocked from subsequent registration if forms are not turned in A fee may be assessed for forms turned in after the first day of class ALL students admitted in or after Fall 2009 must have health insurance. 8. Meningitis Shots Offered Students living in residence halls must be vaccinated against bacterial meningitis or must sign a waiver Call 301-314-8184 to make an appointment if your student would like the vaccine ALL students admitted in or after Fall 2009 must have health insurance.
