Occupational Safety Ice Cleat Program: Providing UAF employees with free ice cleats (traction devices) to help prevent slips and falls on ice/snow Remote Travel Emergency Plans (RTEP): Assisting in developing and reviewing UAF RTEPs which help ensure a safe trip afield Job Hazard Analysis (JHA): Providing specialized training and assistance to develop JHAs which identify workplace hazards and protective measures

Accident – Injury Reporting Report any instance of personal injury, regardless of severity Examples include: cuts/lacerations, injuries from slips and falls, chemical exposures, allergic reactions, muscle strains, repetitive motion/ergonomic injuries, debris in eyes Employees must use the UAF Accident/Incident Report found at http://www.uaf.edu/safety/ http://www.uaf.edu/safety/ DO NOT use the UA Incident Report on UA’s website Complete the report and send to EHSRM within 4 working hours of the injury event Report student and visitor injuries directly to the System Office of Risk Services Claims Department at 907 786-1716

Call 911 when injuries require prompt medical treatment Immediately report to EHSRM any accident with injuries that results in transport for medical treatment or results in known or potential overnight hospitalization Overnight hospitalization must be reported to EHSRM no later than 8-hours from the time the accident occurred Accident – Injury Reporting Failure to notify EHSRM of an overnight hospitalization within 8-hours of the accident can result in OSHA citations and penalties (fines) to the department!
