6 Responsible for providing comprehensive religious ministry and pastoral care to patients and their families. pastoral care including individual and group counseling, leading worships and ordinances, and patient/family education. The Chaplain's duties include but not limited to:  Providing needed attention to families and patients, during times of crisis,  be attentive to the strains and stresses experienced by staff members  providing spiritual support families and patients involved in crisis  work with patients and their families  conduct spiritual assessments,  conveys assessment to other professionals  spiritual counselor to patients and their families on ethical and moral matters, including problems of conscience and family and marital differences  opportunities for free exercise of religion,  locating other chaplains and/or persons from the community to provide for patients' services,  provide those rites and sacraments  Collaborating with other health care professionals  assessing patient needs, planning and carrying out patient care; developing relationships  educating health care staff and trainees about the role of chaplains and the importance of religion and spirituality in health and wellness  an ecumenical, team approach  Maintain an awareness of spiritual needs associated with various medical conditions.  Establishing periods for public worship, conducts weekly/regularly scheduled services including devotions, sermons,  Arranging for special services including administering ordinances, rites or sacraments, conducting funerals, or seasonal services.  Maintaining statistical records, and reporting

8  locating other chaplains and/or persons from the community to provide support for patients' services, - Coordinated military and community organizations in support of deploying units; collaborated with resources to provide training, information, and Veterans services. -Built and maintained relationships with local Pastors and service providers in the community.  Arranging for special services including administering ordinances, rites or sacraments, conducting funerals, or seasonal services. - Prepares and presents Chapel services for service members during training events; performs and provides Worship Services, devotional, rites and sacraments.  Collaborating with other health care professionals -Provides daily individual crisis intervention, care and counseling to Soldiers and family members; works in concert and collaborates with the Military Psychologist, Suicide Prevention Team and Sexual Assault Prevention Team and the Vet Center Counselors to assist Soldiers and families in need. - Counseled and assisted over 40 Soldiers struggling with suicide ideations.  Provide those rites and sacraments  Assessing patient needs, planning and carrying out patient care; developing relationships - Assessed Soldiers’ spiritual needs and provided Worship Services, devotionals, sacraments and memorial services for 2 fallen Soldiers.
