12 Safe Maintenance Rules HAZARD ISOLATIONS

August 6, 2015 | 2 To protect the persons who intervene on a facility, to be sure of the energy absence (electricity, fluids, chemicals and mechanics) The incidents linked to an isolation absence are not numerous but are generally of a strong severity This is an essential step in both the workplace and at home WHY THIS RULE? HAZARD ISOLATIONS

August 6, 2015 | 3 RULE DEFINITION HAZARD ISOLATIONS When performing a task on a powered or pressurized system ensure that isolations have been validated by the exploitation team and double check all isolations are in place as per the isolation plan.

August 6, 2015 | 4 HAZARD ISOLATIONS Exploitation Maintenance Efficiency Corrective maintenance or Preventive maintenance or Ameliorative maintenance Responsibility Time Responsibility transfer Efficiency 1 RESPONSIBLE FOR EACH STEP Responsibility transfer

August 6, 2015 | 5 HAZARD ISOLATIONS Sources and forms of energy that must be isolated: Electrical energy Mechanical energy Water power Pneumatic energy Fluids Ionizing sources Residual energy (stored energy)

August 6, 2015 | 6 Check isolations prior to working on any powered or pressurized system Isolation protects people and facilities against the consequences of: electrical, mechanical energy and pressure energy risks Wear all prescribed PPE PPE is my last layer of protection against electrical risk. PPE is provided by Dietsmann and must be kept in good condition. I MUST HAZARD ISOLATIONS

August 6, 2015 | 7 Perform an absence of power and/or pressure test prior to starting my task To be sure of the energy absence: electricity, fluids, chemicals and mechanics Ensure that the electrical rescue kit is available in the electrical cabinet The kit consists of: a pair of insulating gloves, insulating stool, insulating pole and insulating rug. A safety poster with what to do in case of electrocution is located on the door I MUST HAZARD ISOLATIONS

August 6, 2015 | 8 Work on systems that are powered and/or pressurized Absence of electricity SafetyAbsence of mechanic energy Absence of pressure Perform works on energized systems without a dedicated Permit to Work Isolation is integrated in the Permit To Work Without authorization, I must not intervene. I MUST NOT HAZARD ISOLATIONS

August 6, 2015 | 9 1 Separation2 Lock out & Tag out 3 Dissipation of accumulated energy 4 Recognition5 Check HAZARD ISOLATIONS AN EFFICIENT ISOLATION FOR A SAFE TASK 5 STEPS

August 6, 2015 | 10 HAZARD ISOLATIONS RESTART THE MACHINERY When restarting the machinery, here are some steps to follow: Ensure that nonessential items are removed from equipment Ensure that equipment components are intact Check area to ensure that all affected employees are safely positioned or removed from the area Notify all affected employees and site supervisor before reenergizing the equipment Remove lockout/tagout devices Reenergize equipment to ensure safe operation

August 6, 2015 | 11 HAZARD ISOLATIONS ISOLATION EXAMPLES

August 6, 2015 | 12 HAZARD ISOLATIONS ISOLATION EXAMPLES

August 6, 2015 | 13 HAZARD ISOLATIONS ISOLATION EXAMPLES Bad practice Best practice Opened cabinet Bare live parts Fuses left available No signals Closed cabinet Visible position switch Laying padlock locking Signals

August 6, 2015 | 14 HAZARD ISOLATIONS YOUR LIFE IS AT STAKE ! LOCK OUT & TAG OUT MY LIVE IS AT STAKE

August 6, 2015 | 15 HAZARD ISOLATIONS LOCKOUT / TAGOUT SAFETY ISOLATE ALL ENERGY SOURCES BEFORE MAINTENANCE!

August 6, 2015 | 16 HAZARD ISOLATIONS
