Potential Action Items Please prioritize your TOP 5 goals. 1.To be open (increase hours of operation) full time, 5 days, maybe 1 weekend day, to support those who work during the week (A.2) 2.Implement a formal intake process, utilize a method of assessment to help families ID their resources/strengths to handle a crisis (B.1) 3.Offer programming specific for working males in the community. (B.3) 4.Have parents facilitate our monthly parent engagement meetings instead of staff. (B.3) 5.Educate Marketing Committee on diversity of our community so marketing material reflects the trends in the diversity of the population served. Program partners with families to set goals. It formally facilitates families’ access to services and activities, and has a system to provide them with the follow-up support. Program analyzes results to inform program planning efforts and makes appropriate modifications.
