2 Madison Plains High School Students earn both high school credit and college credit for the same course.

1. Fill out the Dual Enrollment Application. 2. Turn it in to the Guidance Office by September 8, 2011 at 3 pm. 3. Await acceptance letter from the University. 6

1. Fill out the class registration form and submit to the Guidance Office by September 29, 2011 at 3 pm. 2. Tuition will be billed from Urbana University at $225 per class. First semester class payment will come due in November; Second semester and year long class payments will come due in March. 7

Qualified instructors must have a degree/certificate/licensure in the content area of the Dual Enrollment course as well as a Master’s Degree. The Madison Plains class must align with an Urbana University course offering and the class syllabus must be approved by the University. 8

A fund has been set up to help defray the costs of these classes. It is funded by donations and fundraisers. We are a 501(c)3, therefore tax deductible. Checks should be made payable to Madison Plains High School with Dual Enrollment written in the memo. Donations should be given to Mr. J. Henry. The amount of reimbursement is based on the amount available in the account in May and is divided by the total number of qualifiable classes taken. 13

In order to qualify for the scholar reimbursement, a student must: Maintain a 3.0 GPA Have Earned an A or B in all Urbana classes Miss no more than 3 days per semester without medical/legal documentation Have no Saturday school, suspensions or other behavior referrals. Apply for the reimbursement in May per announcements 14
