4 Preference is given to children who have at least one sibling who is enrolled Sibling Preference Definition Sibling means: Children who have at least one common parent or legal guardian AND Live in the same household at least 50% of the time.

6 If your child is accepted we will need the following items submitted to the office within 4 weeks: 1.Official Release of Records Form (ORR) 2.Admissions Form 3.Birth Certificate (Certified Copy) – the school will make a photocopy for their files and return the original to you. A certified copy can be obtained at the County Clerks Office in the county in which your child was born (photocopy, hospital, or footprint copy not acceptable). 4.Immunization Records -- Photocopy of child’s most recent immunization records. Without these records, your child may be removed from school. 5.Proof of Custody -- If parent names on the birth certificate do not match the person signing the Admissions Form, a court order allocating parental rights, marriage certificate if a step-parent, or a Caretaker Authorization Affidavit if a grandparent is required (form available at school). 6.Proof of Residency -- Current mortgage or rent payment receipt; lease agreement; voter registration card; gas, water or electric bill (no phone bills accepted); or bank statement. 7  Movement on the Waiting List is very common.  If you are serious about having your child attend the school, you should remain on the Waiting List.  Openings can occur any time, even after school begins.  You will be notified when a seat becomes available and will be given a timeframe in which to accept this spot.  If you choose not to accept the seat, your child will be taken off the list. 8  The Standby Opportunity Plan is a procedure that may better your position on the Waiting List after the first day of school. One to two weeks before the first day of classes the school will send a letter explaining how to participate in this procedure.  Removal From Waiting List -- Please tell the school if you want your child’s name removed from the Waiting List as soon as you make that decision. 9  It is your responsibility to notify the school of address or phone changes  If we try to contact you by mail and the mail is returned, or if we are unable to reach you at any of the phone numbers you provided, we will remove your child’s name from the school’s list Address Changes
