11 Check if all Districts / facilities have filled in the Data for the Month Check for the Completeness of Data.. Percentage filled Check for Validation errors Check for Outliers Feedback to districts/ sub- districts Forward Data Required Process (monthly from facility, district, state) If not followed gives erroneous data

12 Facility Based Reporting Online data entry, directly in HMIS portal Data entry in Offline excel sheet and then uploading in HMIS portal Excel files can be downloaded from the downloads section (HMIS formats) The Excel sheets is encrypted and does not allows the user to change, delete or modify any items or cells other those nominated to enter the data.

13 Downloads Section- download the Performa pertaining to your facility, and save to your disk.

16 Data Uploading - Ways 1.Single facility Data Upload 2.Multiple Facility Data Upload New

17 Process - Single facility Data Upload Login with your username and password. Go to MIS-> Monthly Click Add Select year, month, proforma / format and facility name click on Move Next.

18 Process - Single facility Data Upload (contd.) Upload the offline already filled excel sheet by clicking the upload proforma / format link Fill the online format for online data entry and press the update button

31 Multiple facility data upload Data of Multiple facilities is uploaded in one go by using offline excel sheets User can upload data for 25 facilities at once The data is updated on the portal within 24 hours If data for facility is not uploaded then an error message is displayed indicating reason for not uploading.

32 Process - Multiple facility Data Upload Login with your username and password. Go to menu offline data upload Click Add Select year, month, Performa / format and facility names Browse the required files in front of required facilities Click on Upload Excel

33 Multiple facilities may be choosen Choose location of file to be uploaded * Make sure that the data uploaded matches the facility

34 Data Forwarding After uploading,the facility data has to be Verified using verify button The sub-district user has to then forward the facility data Once forwarded the data cannot be changed until it has been reset to draft mode

41 Data Forwarding Data can be forwarded from sub-district via: 1.Individual file forward 2.Bulk Forward
