4  Child may be exempt for medical reasons when medical provider deems necessary for exemption  Child may be exempt for religious reasons from immunizations when parent object by a written sworn statement to the immunization of the child on religious grounds

5  Must be current (no expired certificates)  Presented to school within 2 weeks of enrollment (702 KAR 1:160)  Neither statute or regulation assign “penalty” if certificate not presented  Local board of education policy determines “penalty”  Homeless children protected by McKinney-Vento act and cannot be excluded from attending school
