2 Figure 51A What do you see by light microscopy (Jones’ silver stain, original magnification ×200)? There is mild mesangial proliferation with increased matrix, mild prominence of glomerular basement membranes, and mild to moderate afferent and efferent arteriolar hyalinosis with a capsular drop. There is also mild interstitial fibrosis. These findings are consistent with mild diabetic nephropathy. The presence of mesangial hypercellularity without Kimmelstiel-Wilson nodules suggests an early stage of the disease. 3 Figure 51B What additional findings do you see by light microscopy (hematoxylin and eosin, original magnification ×400)? Further examination revealed focal areas with interstitial edema and numerous eosinophils, in addition to lymphocytes and plasma cells and a vague granulomatous reaction. 4 Figure 51C What additional findings do you see by immunofluorescence microscopy (anti-IgA antibody immunofluorescence, original magnification ×200)? By immunofluorescence, there was 1+ mesangial staining for IgA, trace mesangial staining for C3, and very trace mesangial staining for κ and λ. Stainings for IgG, IgM, and C1q were negative. 5 Figure 51D What do you see by electron microscopy (electron microscopy, original magnification ×5,600)? Electron microscopy showed mild increase in thickness of the lamina densa of the glomerular basement membrane, increased mesangial matrix and cellularity, and small- to medium-size mesangial immune complex deposits (arrows).
