3 Safety Forms There are 4 forms available for staff to use to report incidents or to identify areas of improvement to ensure that the workplace is safe Near Miss – No Name, No Blame Incident Investigations First Aid Injury Form Continuous Improvement Forms Question: Where can you find these forms in the Administration Office?

4 Near Misses No Name, No Blame Forms:
..\..\..\Forms\Near Miss Report No Name No Blame.docx A near miss is an unplanned event that has the potential to cause human injury, environmental or equipment damage, or an interruption to normal operation. Question: Can you provide an example of a near miss?

5 Incident Investigations
Incident Investigation Forms: ..\..\..\Forms\Incident Investigation Report form.doc When to use these forms: Accidents that result in injuries that require medical aid Accidents that cause property damage or interrupt operation with potential loss exceeding $500 All incidents that, by regulation, must be reported to OH & S, WCB, or other regulatory agencies. 6 First Aid Injury Form First Aid Injury Form:

Stabilize - Enforce a place for everything and everything in its place. Shine - Clean up the workplace and look for ways to keep it clean. Standardize - Maintain and monitor adherence to the first three Ss. Sustain - Follow the rules to keep the workplace safe. Safety - Eliminate hazards.
