4 UCRs A Code: Example Hauling company has a 22 tn minimum 30 tons hauled to soil treatment facility Hauling company bills 44 tons (30 tons plus 14 tons) to achieve 22 ton minimum Pre-approve hauling 30 tons using T075 & T076 Additional 14 tons listed as A-code (A001) Invoice from hauler identifying minimum charge must be submitted with claim Invoiced amount plus up to 18.5% eligible for reimbursement

11 UCRs Mark-up The difference, as a percentage, between the cost of an item or service and what the client is charged or claimed from the Fund Mark-up is limited: A code - 18.5% C code - 6.0% X code - 16.0% Bids - 16.0% UCRs for T & M codes include mark-up

12 UCRs Laboratory & Soil Treatment Costs Base costs are diverse Incentive for continued cost savings that the program can share Savings Pass Along Invoiced amount plus one-half the difference between the UCR and the invoiced amount Lab and soil treatment costs will be reimbursed: Invoiced cost + 18.5% the UCR, or Invoiced cost + ½ (UCR – Invoiced cost) UCR

13 UCRs M1376 – UCR $91.56/sample Lab invoiced cost is $60 + 18.5% = $71.10 max allowable reimbursement without Savings Pass Along Savings Pass Along = Cost + ½ (UCR-Cost) $60 + ½ ($91.56 - $60) = $60 + $15.78 = $75.78 Claim $4.68 (6.5%) more than $71.10 Fund saves $15.78 over the UCR

14 UCRs T012 – UCR $32/ton Treatment facility invoiced cost $16 + 18.5% = $18.96 max allowable reimbursement without Savings Pass Along Savings Pass Along = Cost + ½ (UCR-Cost) $16 + ½ ($32 - $16) = $16 + $8 = $24 Claim $5.04 (32%) more than $18.96 Fund saves $8 over the UCR
