1.  HSE LEARNING INCIDENT: CAUSES: LEARNINGS:
Derrick man was helping the installation of the escape line pole in the monkey board of the hoist. His finger got caught between the pin and the steel floor edge causing crush injury.
CAUSES:
Wrong hand position
Improper risk mitigation to install the post
Unexpected pin movement
LEARNINGS:
Need to redesign and reposition the post
Proper risk assessment to be done
Sub-optimum design could led to catastrophic incident
Struggling to inserts the Pin 
Key Learning:
Report any difficulties in completing task due to poor design
