1. DMDC Online Access To obtain access to the DMDC Online you must:
Download System Authorization Access Request (SAAR) DD Form 2875 (Aug09) located on this webpage or can be downloaded at:
Be sure NOT to use SAAR-Navy (OPNAV 5239/14)
Fill out it out following the directions on the next slide.
Ensure it is digitally signed by you, your supervisor, and your Security Manager.

*If the form is not sent encrypted – NMCI will strip out the digital signatures and you will have to re-submit it.

2. DD Form 2875 Complete the following fields at the top of the form:
- Type of Request: Initial Date: Enter today’s date
- System Name: TAP Web (App ID 618) Location: Seaside, CA
PART I: Fill in Blocks Make sure you put the date in Block 10, and Digitally Sign Block 12.
PART II: Fill in Block 13. Enter your TYCOM/ISIC Command (Upper echelon command). Add your justification of why you need to be able to do in DoDTAP for Managers and Counselors.
(ie: Needs ability to log into the TAP web application to enter or retrieve official DD Form 2648 records, manage attendance data for TGPS courses, access to VMET documents and view reports.)
the form to your supervisor and request they complete the following fields and return back to you:
- PART II – Blocks 14 (Authorized), 15 (Unclassified) , 16, 16a, 17, 18 (Digitally sign), 19, 20, 20a, 20b
- Leave Blocks blank; this will be completed by the SSM when you submit your form.
the form to your Security Manager/Information Assurance Officer (IAO) and request they complete the following fields and return back to you:
- PART III: Blocks 28, 28a, 28b, 28c (must be either II or III), 29, 30, 31 (Digitally sign), 32
(* Note for Block 28a: TAP contains PII, and therefore requires a IT-II or III Level Designation.)
