1.   HSE LEARNING INCIDENT:
Deceased died in his room due to a internal bleeding leading to heart attack
CAUSES (Contributory Factors):
High blood pressure
Diabetes
Intake of alcohol
Latent management cause – Not ensuring the program medicals was followed
LEARNINGS:
Ensure ALL personnel have an in-date medical declaring fit to work
Follow up any findings identified in medicals  
Key Learning:
Check for Chronic disease and apply Control measures
Non Accidental Death
Use this Alert: Discuss in Tool Box Talks and HSE Meetings  Distribute to contractors  Post on HSE Notice Boards  Include in site HSE Induction
