1. Identify Lab Site for clinical skills training 2. Identify facility instructor 3. Fill out Facility registration form 4. Fill out administrative portal form 5. Fill out course build form 6. Complete orientation training session 7. Start registering students in portal 8. 2 Step One Facility Approval of Nurse Aide training and competency evaluation program Identify a qualified facility in your area Or consider becoming a certified facility if you meet state requirements Contact Julie Gdowski if you want to become a qualified facility & conduct laboratory and supervised clinical training in your facility Ensure you have the required equipment Create schedule for class identifying lab days and coordinate with other administrators if multiple sites are involved Available Master Level University Instructors On-Line Certification Course for Preceptors Employed or contracted options Must meet all state requirements (i.e. minimum RN)

3 If your facility has an approved program already you need to make sure:
The RN approved to teach the program is still the same RN or you will need to get approval from governing body for the current RN. You will need to send a syllabus outline of the AHCA blended on- line program and sample calendar of the blended on-line program. Contact Julie Gdowski if you do not have this information.

4 Instructor Selection and Registration
Step Two Instructor Selection and Registration Identify an instructor that has completed state required training or meets requirements Approved Instructor should then register for a subscription to the Instructor Online resource site(this is not the course, it is resources for teaching) Purchase Samsung tablet for online Competency check off grading if you do not have a tablet or PC already available Available Master Level University Instructors On-Line Certification Course for Preceptors Employed or contracted options Must meet all state requirements (i.e. minimum RN)

5 Facility Registration
General Information Step Three Contact Gwynette Winstead to let her know you want to host a class she can help you with the following forms Fill out the Facility registration (Available online) Available Master Level University Instructors On-Line Certification Course for Preceptors Employed or contracted options Must meet all state requirements (i.e. minimum RN)

6 Fill out Portal Step Request form Four Complete the Portal Form
(Available online link below) After several days you will receive notification that your portal has been created from CNA online and instructions on how to access it Available Master Level University Instructors On-Line Certification Course for Preceptors Employed or contracted options Must meet all state requirements (i.e. minimum RN)

7 Complete Course Build Form
Step Five Complete Course Build Form Fill out course build form (available online) Julie Gdowski can help with this form. 8 Complete Orientation Training Session

9 Access your new Portal and register students
Step Seven Log in to your new portal per instructions sent to you from CNAonline (Our team will walk you through this or we can do this for you) Start registering new students per instructions (Our team will do this for you if you prefer)

10 Congratulations, you are ready to launch
Step Eight Finalize Class Launch Confirm schedule for class and make sure all instructors, administrators, and students are aware of lab days and location prior to submitting final schedule Have your selected instructor contact our nurse educator Julie Gdowski and let her know you are ready to finalize and launch your class. Julie should be notified at least 3 weeks prior to your actual class launch date. We are ready to get started! Congratulations, you are ready to launch your first class!!!
