4 Filing an Individual Application
Go to Choose – Join Now – from the headers Choose – Individual Enrollment - from the drop down menu Click the box marked “Individual Enrollment Form” Complete the form as requested. (Note if you do not have a FOP Member Number from your Lodge/Grand Lodge – please enter the word “Pending”. If you do not know your Lodge No., please contact your Lodge representative to request the Lodge No.) Choose your Payment Method (depending on method there will be additional options) Complete the information requested regarding your employer and certification to carry firearms Sign (by typing your name) and Date Click – Continue to Payment Page. You will receive further information about process of payment (if via Check you will be provided an address to mail payment, if via Credit Card you will be provided information for payment via PayPal.)

5 Filing a Group/Lodge Application
Go to Choose – Join Now – from the headers Choose – Group Enrollment - from the drop down menu Click the box marked “Group Enrollment Form” Complete the form as requested. Choose your Payment Method (depending on method there will be additional options) Complete the form for your Member Listing OR your Lodge Roster with all required information (.XLS format only - Excel) Sign (by typing your name) and Date Click – Continue to Payment Page. You will receive further information about process of payment (if via Check you will be provided an address to mail payment, if via Credit Card you will be provided information for payment via PayPal.)

6 Filing a HR-218 Application
Go to Choose – Join Now – from the headers Choose – HR-218 Enrollment - from the drop down menu Click the box marked “HR-218 Application” Complete the form as requested. (Note if you do not have a FOP Member Number from your Lodge/Grand Lodge – please enter the word “Pending”. If you do not know your Lodge No., please contact your Lodge representative to request the Lodge No.) Choose your Payment Method (depending on method there will be additional options) Sign (by typing your name) and Date Click - Continue to Payment Page. 7 Filing a Claim Online Go to www.foplegal.com
Choose - Plan Attorney & Claims – from the headers Choose - Claim Reporting Form - from the drop down menu Complete the Claim Reporting Form, then sign/date, and click Submit Claim Report (at the bottom of the page). You will receive a pop-up box that says “are you sure you want to submit this claim request?” Choose “OK” You will then receive a notice that you will be contacted shortly by a Claim representative. If you do not receive this notice or are not contacted within 24 hours, please call –

8 Accessing the Client Portal
The client portal was developed to assist groups or lodges in maintaining and accessing their accounts. There are many options available such as adding, editing and deleting members, and viewing account statements. Once changes are made to your account, they are submitted to Hylant for final approval and then entered into our database. If your lodge has a group membership with the FOP Legal Defense Plan and is interested in participating in the Client Web Portal, please submit a Client Portal Registration Form. Please note, member reinstatements cannot be submitted via the Client Portal. Please submit the request in writing to Hylant. 9 Using the Client Portal
Once you have your username and password (see Accessing the Client Portal) – visit - Login using your username/password combination NOTE – to add/delete/change membership roster information the Lodge will need to be added to the coverage first. Please contact Hylant at for more information about adding your Lodge to the coverage. Once you have accessed your Lodge information Click on the Header – “Member Roster” To add a member – click the “plus” icon and add the member information as required. Then click add. To remove a member – click the “x” icon to the left of that member’s name in your roster. To change a member’s info – click the “pencil” icon to the left of that member’s name. Make changes, then click update.
