1. To start Affiliation of your members go to www.hosa.org
Click on Advisors then Local Advisor

2. Click on On-Line Affiliation

3. After you have read all of the Instructions Click on Affiliate

4. Enter your Charter Number, this number is a 5 digit number starting with a 6

5. Click to Proceed

6. After reading instructions, click on Complete the Affiliation Application

7. After reading the instructions, click next

8. Fill out all information, then click next

9. Fill out all information, then click next.

10. Fill out all demographic information
Fill out all demographic information. This is important information that we use when gathering data for grants and partnerships.
If you don’t know this information, you can log back in and complete it later.
Click next.

11. After reading the instructions start adding your students’ information.
It is helpful if you gather this information before you start affiliating your students

12. You will see how many total members you have affiliated

13. When you are ready to complete the affiliation process, click done.

14. Make sure to log back in to the affiliation site to print your Account Statement and Membership Directory
Affiliation fee due date will be on your account statement along with the address of where to send fees
If after clicking on “done”, you log back in to add more members, you will not see the list of members that you have already entered. Don’t panic! Those students are in the system. Do not re-add them. Doing so will duplicate your members.
After adding more members, make sure that you log in and print your updated account statement again
