3. Poster Contest Entrant Media Release Form
Our school is having a Digital Citizenship Poster Contest and your child is eligible to enter.
When the winners are selected, we would like the opportunity to publicize his/her work both in print and online. With your permission, your child’s full name, the name of his/her school, and an image of his/her work may be published in the press, used for commercial purposes, and/or displayed on the internet. This is a wonderful opportunity for your child to receive recognition for his/her work.
A signed release form is required for every student whose name/image/likeness is printed or posted online.
If you choose not to sign this release, we will simply list the name of the school and an image of your child’s work without mentioning his/her name.
If you would like your child’s work to be credited, please fill out the form below.
