1 HSE LEARNING INCIDENT:
While the IP “Chief Mechanic“ was in the process of checking and recharging Pulsation Dampener on Mud Pump #2, he lost his balance and fell on the side of mud pump and sustained injury on the right side of the head. CAUSES: Slippery work area due to use of OBM Fixed or Mobile platform was not used T/P didn’t inspect the work area prior to issue PTW IP age was 62 years LEARNINGS: Clean Work Area Prior to start job, specially when use of OBM Use JSA/ Procedure prior to each job Use fixed or mobile platform when working on pulsation dampener Tool Pusher to inspect work area prior to issue PTW Employees above age 60 shall not be allowed to work in the field Key Learning: Beware Of Slippery Work Area When Drilling With OBM Head Injures Use this Alert: Discuss in Tool Box Talks and HSE Meetings  Distribute to contractors  Post on HSE Notice Boards  Include in site HSE Induction Contact: MSE511 for further information or visit the HSE Website Alert No 94 LTI # September 2012
