What is Kaiser Permanente?
Kaiser Permanente is an integrated delivery and financing system comprised of three entities that operate through contracts with one another: the Kaiser Foundation Health Plans, Kaiser Foundation Hospitals, and the Permanente Medical Groups. We sometimes say " the patient is the most important primary care provider" We build into our systems ways of identifying and supporting our patients to support healthy lifestyles and help them manage their chronic conditions. We build trainings and resources using motivational interviewing and brief negotiation theories. These rely on the patient to identify their core values and motivation to craft their own strategies for healthy changes In outpatient clinic visits when the patient is lead to the exam room, we ask questions as part of our "Vital Signs" -- Do you smoke? How many days do you exercise and for how long? How much alcohol do you drink a week on average? This presents opportunities for physician conversations and referral to other resources We do both outreach -- identifying patients who need preventive services and contacting them -- and in-reach -- offering patients who are in our offices services like mammograms on the spot We can refer patients to a large variety of resources to support healthy living -- in print, in person and on line classes, one on one counseling, wellness coaching and more. In NCal, our robust physician home pages, My Doctor Online [kp.org/mydoctor] include excellent, physician vetted information, interactive online tools, and videos that help support the patient's health journey

Tobacco Cessation Approach –
KP developed a comprehensive tobacco cessation & prevention strategy that includes: Systems integration: treatment decision support and screening reminders in the EMR Evidence-based patient education and behavior change support through coaching, classes, and online tools (no additional costs or limits for members) Outreach strategies to monitor patients, prevent relapse, or re-connect with support Clinical guidelines and training support Social marketing campaign (50,000 Quitters) to engage patients and staff Outcomes – Over 100,000 Quitters in past 4 years Steady decrease in prevalence (KP population is approximately 27% below state prevalence. DOR Study: Quit Rate 31% vs Control 23%
Wellness Coaching participants quit tobacco and filled tobacco cessation medication prescriptions at a comparable rate to tobacco cessation class participants and a statistically significantly higher rate than a matched control group.

Also provided in Spanish. Coaching is based on motivational interviewing based conversations which helps to evoke internal motivation, affirm past success and attempts to change, assess readiness for change, and co-creates a realistic plan. Since 2010, we have coached 26,000 patients in 65,000 visits.

Supporting Members to Lead Happier, Healthier Lives
All KP Medicare and commercial plans were rated highest (or tied for highest) in all areas we serve in NCQA study. In 2017, KP members in 6 geographic areas rate KP highest in satisfaction in the J.D. Power Member Health Plan Study. Also, KP members rate KP highest in 2016 JDP Medicare Advantage study. 15 Kaiser Permanente facilities honored in 2015 as ‘Top Performer’ for Quality and Safety by The Joint Commission. Accolades: Supporting Members to Lead Happier, Healthier Lives In 2017, for the 7th year in a row, KP has earned the PQA’s Excellence in Quality Award for medication safety and appropriate use. In 2016, for the 9th straight year, KP is the only plan in California to earn the 4-star rating for overall quality of care in OPA’s Healthcare Quality Report. In 2016, 37 KP medical centers continue to be Stage 7 Award winners, recognized for successful electronic health record implementation Five Kaiser Permanente regions received 5-Star ratings for 2017 Medicare plans, the highest possible rating. In 2016, for the 4th year in a row, KP’s California health plans received top rating for quality and service. In 2017, for the 7th year in a row, Kaiser Permanente received Satmetrix’s highest customer loyalty ranking in the health insurance category. Is it a coincidence that Kaiser Permanente has been ranked highest in customer satisfaction for eight consecutive years (according to the J.D. Power Health Plan Member Survey) – the same eight years that members have been able to use our online features? In 2015, KP was honored with Leapfrog’s inaugural “Living the Vision” award for excellence.
