6 Open Space A free platform for you to discuss any topics of interest and disability issues that concern you Common themes and popular topics will form the subject for the next Focus Group meeting and CCG consultation topic.   Some possible themes are accessible venues, housing issues, health and wellbeing, sport and leisure, getting out and about…. Or something completely different!

12 Health and Wellbeing visits – what we do
Arrange for older people to be visited in their own homes for a conversation with the volunteers about their health and wellbeing needs and priorities Offer basic information and advice together with the offer of a referral to an appropriate source of help Volunteers do a brief fire safety check and give advice about how to stay safe at home Evaluate each visit and offer a follow up visit to check the progress of any referrals Trained volunteers Over 50 with long term condition, disability or is socially isolated 17/01/2019 12

13 Health and Wellbeing visits – aims
Vulnerable older people will be more aware of their health needs and their choices of public and community services Vulnerable older people will be taking more action to manage their health better Vulnerable older people will feel more socially connected Reducing health inequalities Build an effective referral network so that we can help reduce our partners cost and time spent on addressing vulnerability Develop a model which can be used Nationally 3 year project funded by Department of Health Learning project 17/01/2019 13
