When completing your electronic enrollment agreement, it is essential that all documents have your electronic signature, date, and initials (where applicable). A complete enrollment agreement ensures a smooth and timely enrollment process.
You must double check and perhaps triple check all documents before submitting them to your Medical Career Specialist. If a document that is incomplete, your Medical Career Specialist will notify you.

Let’s scroll down to the Conditions of Payment section, initial this field and fill out the other fields.
Let’s go to the second page and scroll down. Don’t forget to initial the bottom of the second page.
You must request that a copy of your transcripts be sent to the Pima Medical Institute Online Education Department. The address is 40 N Swan Road, Suite 100A, Tucson, AZ Attention to: La Trina Merritt/Registrar. It is your responsibility to make sure the files can be clearly read before you send them! Failing to do so will cause delays.
On the Attestation page, indicate whether received your high school degree or your GED with your initials, not an X or check mark.
If you are in the Advanced Placement Track Program, pay attention to the Required Travel Notification form. Do not leave any fields blank. Indicate your order of preference in numerical order. Your most desired option is number one and your least desired option is number 5.
