2 Welcome and introduction
We want to listen to you, to work with you, to design health and care services that are fit for the future We will explain why services need to change This event is the first in a number of conversations, the start of a new relationships between our communities our services and statutory agencies

To listen and learn about what is important to you To gather people together to share information and explain how people can get involved To gather your views on local services

5 Our Health Our Care As a programme, our purpose is to:
Design a new model of health and care to ensure that we can meet your needs, no and in the future Our services are clinically and financially viable for the future We will do this together through an ongoing relationship

By working with our neighbours, we can share good ideas and work through common challenges together Contributing to Local change programme

7 Why is Change needed? We need change to ensure that people can stay healthier and happier for longer. Animation on the Case for Change

8 Why is Change needed? We now want to look in more detail at some of the key facts in the case for change, at how some of our demographics are changing, and what this means for our communities and demands on services

9 Why is Change needed? Our local population is growing and living longer 3.6% 2016 Local Population Growth Dementia is forecast to increase by 18% over the next three years

Why is Change needed? Chronic conditions Increasing Long-term conditions 2018 1m 3+ long term conditions Across all central Lancashire, rates are higher than the England average

11 Why is Change needed? Preventable Illnesses are increasing
Greater Preston has a significantly higher prevalence of smoking and rates of admissions to our hospitals for attributed diseases is significantly higher than England Leading to an increase in largely preventable conditions like type 2 diabetes, heart and liver disease Over 18s alcohol related hospital admissions have increased significantly over the last 10 years in Greater Preston and South Ribble

Why is Change needed? Variation in health and wellbeing There is variation between locations and services which we need to change Deprivation . 1 in 3 people in Greater Preston live in deprivation 1 in 8 people in Chorley & South Ribble live in deprivation 7 years 10.7 years

13 Meeting increasing demand
Why is Change needed? Meeting increasing demand Our acute hospitals have seen 11,245 more outpatients provided 10,374 more operations and admitted 1,087 more patients …than in 2014/15 Lancashire’s mental health teams handle 2.7 times the national average number of referrals (the highest in England) and caseloads that are twice the national average Our mental health hospitals are operating above 100% occupancy Adult community services in Lancashire have experienced a 30% increase each year in referrals for community based nursing, allied health professional services etc. In terms of social care, there are 26% fewer people over the county who since 2010 get help

Why is Change needed? Our budgets and financial challenges The financial challenge for the health and care economy in our local area is estimated at around £221 million over the next five years. £150m gap

16 Why is Change needed? Our workforce
Workforce challenges have affected a range of services. There is a shortage of doctors, nurses and therapists. Doctors in training and specialists have an impact on vacancy rates, turnover and temporary workers 30% of GPs reaching retirement age in the next ten years 63% primary care nursing workforce reaching retirement age in the next ten years Social Care workforce at county level is under established

Meet Bernard, he is 86 and has several conditions, emphysema, type 2 diabetes and arthritis.

If our services were more joined up, he might have a single contact with overall responsibility for his care

20 So, together we are planning for the future...
We have clinicians, staff and stakeholders coming together to start to think about how we can all work differently. These conversations will continue to make sure that ideas on how things could be done differently are gathered.

22 This is the purpose of the Our Health Our Care programme …
We will develop a new model of health and care for central lancashire that will ensure we are financially and clinically sustainable for the future We will ensure the process is clinically led and we coproduce the model with the public and patients We will use the process to design, validate and evaluate options for transformational change and a new model of care for the Central Lancashire Health and Care system

23 We will undertake this work over the next six months …
Sept – Mar 17 Spring/Summer 2017 Solution Design Consultation Implementation During the Solutions Design Process, we will; Develop Evaluation Criteria, Benefits Framework, Quality Standards Develop new models of care and ways of working across the health and care economy Develop options to deliver new models of care All of this will be in conjunction with our communities through events such as this, and any major changes will be subject to a formal public consultation process

24 In conclusion … The case for change is compelling
To do nothing is not an option. Change is inevitable and some difficult decisions will need to be taken. The Our Health Our Care Programme is the opportunity to harness the energy of our clinicians and communities and transform our health and care system to meet your needs To make real change we need to think radical and differently and to do it together
