3 Prior to Writing a Referral
Before writing a referral, many of the situations can be curbed by sending the student to a “Buddy Room”. Have a conference with the student discussing the behavior. Document all contact in your contact log.

4 - Log into SIS. - Click on the Puzzle Pieces.

5 Type in the Last Name and the First Name of the student receiving the referral. Press “RETURN/ENTER” key

6 This is the screen that will appear.

7 The date and time of the incident are automatically set to the ‘date and time’ of the computer. You may need to ‘adjust’ the time. Do this by clicking in the box and put time of the incident. You are also able to change the date if you need to.

8 In the box below the time and date, type a description of the incident
In the box below the time and date, type a description of the incident. If the student made verbal comments, make sure you quote EXACTLY what the student had said. MAKE SURE YOU SPELL CHECK as this will be visible on the parent portal as well.

9 In the drop down CODE box, select the reason for the referral, if there are multiple reason, use the one that is most pertinent to the referral.

10 Place a check mark in the “Any corrective action prior to this referral”. Change the date to when any prior correction was made (if needed). Type in the time that the correction had occurred.

11 In the box below the date and time, type in the corrective measures that you have taken. Make sure you spell check the corrective measures taken.

12 Back towards the top, click the NEXT button.

13 Your screen should look something like this.

14 Click on the SELECT COURSE

15 Click the BLUE CIRCLE ARROW next to the class that student is in.

16 If you would like to be present for the conference, CHECK that box and provide when you are available (your plan periods).

17 When you are ready to send the referral to the office, CLICK on SEND TO DISCIPLINE OFFICE. The Administration team will be able to pull their students. You do not need to specify which Administrator it needs to go.

This is the screen that will appear next. It has the information that you had typed in and provides the information automatically that the Administration team needs.

19 When you are done with the writing the referral, CLICK “DONE”
When you are done with the writing the referral, CLICK “DONE”. This will take you back to the main screen.

20 If you would like to see the status of the referral, CLICK on VIEW/MAINTAIN and then DISCIPLINE REFERRALS.

21 The next screen will allow you to see which administrator was assigned to it and the action/status taken. You will need to check this as the referral might be sent back to you for action on your part.

22 There may be a time that you will not have time to write the referral immediately. If this is the case, CALL THE OFFICE for a DSO to escort the student. Then call the administrator’s office and let them know what has happened and why the student is coming.

23 REMEMBER Make sure you document all contact you have made with the student and parents/guardians.
