2 LTI Data Book New initiative from the MSE3 team to collate all the info related to incidents under one webpage. It contains the LTIs along with the HiPos and HVLs. NADs are not included because of the sensitive info they contains.

If you want to get more info on incidents where to go? All info is scattered in different places across various web pages. LTI Data Book contains all info you need about the incidents.

5 Near Miss reporting tool

6 What is a Near Miss? It is an unplanned event that has the potential to cause harm -but does not- to human , environmental or equipment. Why do you think reporting near misses is important? How many of you have reported a near miss? Have you used the tool? Why not?

7 The Objectives Reporting Culture Simple Tool Learning Easy Tracking
` Learning Enables PDO and contractor companies to pro-actively eliminate and/or control hazards before a tragic or costly incident occurs. Easy Tracking Reporting and tracking near misses can provide valuable information as to where the gaps in the HSE management system exist. Reporting Culture Employees are encouraged to report near misses without fear of disciplinary action or loss of job. Simple Tool Easy access and user-friendly reporting tool Why the Tool is created?

8 What is a Near Miss? I would like to start my presentation with a quick review of near miss. It is an unplanned event that has the potential to cause harm -but does not- to human , environmental or equipment.

10 Identify the Near Misses?
NM UC UA LWC 1. A scaffold tube fell from height and struck the shoulder of a worker below. 2. A crane travelled under the overhead power line and just missed the live cable. (There is no goal post provided). 3. While cementing 7” casing, the light fixture cover dropped from 7m height to the rig floor and landed 3ft away from the crew. 4. An excavation was left without lights at night. 5. During beam pump installation, the spanner slipped from the worker’s hand and fell on the ground. 6. 14 Principles of TBT Reflective Learning
MSE3 team facilitate the reflective learning tool box talk (TBT) training programme. Taking key people from PDO and its contractors through reflective learning techniques which they will then pass to supervisors in the field in how to deliver a high quality TBT and ensure engagement with their teams in “Learning from Incidents”. By implementing this “real learning” approach we will actively engage their hearts and minds leading to a change in behaviour which will create a safer future for us all. How ? By Employee engagement

INCIDENTs Theme LTI and AIPS incidents since 2014 PDO LTI themes First alerts Lateral learning's / Second Alert’s Industry incidents Out of sight locations Line of Fire Specialist Contractors No leaks, I own my barrier Road Safety No leaks2, I keep my barrier strong Lifting and Hoisting 3 minutes There are two ways of conducting a reflective learning TBT either using a Reflective Learning Incident Card or using an Incident Theme. The TBT incident cards are one page A3 cards which I will show later If discussing a theme, you can use a relevant incident card or just a themed card to instigate discussion. There are currently four themes from the last 2 years, ‘out of sight locations’, ‘line of fire’, ‘specialist contractors’ and ‘no leaks I own my barrier’ And 3 new ones for this year ‘Road Safety’, Lifting and Hoisting If delivering in a class room for a training session then there is also material you can use for training on the themes including videos and support packs CLASSROOM tbt

18 Use hands-off where possible
A floorman, with the help of two colleagues attempted to unscrew and remove the sucker rod (SR) table in order to adjust the SR wiper rubber. As they were removing the table, it slipped from the hands of one worker tilting to one side trapping the floorman’s left hand against one of the stripper screws resulting in a fracture of his left hand little finger. Ensure all incidents are reported Ensure Supervisor’s are supervising Ensure the task is done using hands off techniques where possible Mechanical lifting devices should be used when required Use hands-off where possible كان عامل الارضيه و بمساعدة اثنين من زملائه يقومون بفك وإزالة (Sucker Rod Table) من أجل ضبط الممسحة المطاطية. عندما قاموا بإزالة الطاولة، انزلقت من يدي أحدهم لتميل إلى جانب الآخر لترص اليد اليسرى لعامل الأرضية ضد احد المسامير مما أدى إلى كسر في الاصبع الصغيره لليد اليسرى. لا تستخدام اليدين حيثما أمكن تأكد أنه تم الإبلاغ عن جميع الحوادث تأكد أن المسؤول يشرف على العمل تجنب استخدام الأيدي حيثما كان ذلك ممكنا يجب استخدام أجهزة الرفع الميكانيكية عند الحاجة
