1 Preliminary Draft 5 Year Action Plan HIV, Hepatitis C, and STIs
For Presentation at Know Your Status Forum February 14, 2018

2 Background Very high rates of HIV, Hepatitis C, and sexually transmitted infections preliminary action plan has been drafted by Chiefs, Council Members, Health Directors, Elders, Community Health Nurses, other front line staff, and people living with HIV and Hepatitis C from Ahtahkakoop Cree Nation, Big River First Nation, and the Saskatoon Tribal Council. Other contributors were the Federation of Sovereign Indigenous Nations and First Nations Inuit Health Branch - Saskatchewan Region On November 2, 2017 this plan was further reviewed and developed at a meeting of leaders and staff from several communities, Tribal Councils, and NITHA which was held at Wanuskewin Heritage Park intent that the reach of this action plan extends beyond these current member nations, to all interested First Nation communities in Saskatchewan

4 The Action Plan Six major objectives are suggested as structure for the actions needed Community Engagement & Readiness Prevention – Education and Harm Reduction Testing Treatment as Prevention Client Support and Case Management Surveillance, Evaluation and Research Management Activities are to be identified for each of the objectives

5 1. Community Engagement & Readiness
Host an HIV Knowledge Exchange Meeting November 2, 2017 Contact Feb 2017 Forum participants Create a website: knowyourstatus.ca Revise the Know Your Status (KYS) toolkit Host Forum #2 in Saskatoon February 2018

6 2. Prevention - education
KYS toolkit Consistent messaging on HIV, Hep C, STIs, & harm reduction KYS Forum School based education Community and Elder education Share Resources Mandatory STBBI training for all community health nurses and all other health staff in community

7 2. Prevention – Harm Reduction
Promote and support harm reduction includes needle distribution, take home naloxone, and may include opioid substitution therapy (OST), such as suboxone and methadone, safer injection/consumption Encourage needle drop boxes in communities Develop and Share Policy Guidelines for Harm Reduction

8 3. Testing Explore and promote point of care testing options: phlebotomy, rapids, dried blood spot, urine, self (including related policy) Identify and address barriers to lab access and licensing Promote: test for one, test for all

9 4. Treatment as Prevention
community based and community led treatment for HIV and Hep C case management and bridging gaps Pre-exposure prophylaxis medications (PREP) where suitable collaborative approach/universal acceptance of treatment as prevention opioid substitution therapy and other addiction care Improve transportation support Primary care provider competency (re meds) Collecting and analyzing data in community to inform decisions around health services

10 5. Client support and case management
a list of supporting education and competencies for the KYS program develop sustainable funding and longer agreements improve communication and care for clients policy guidelines for case management a network of quality primary care accessible mental health & addiction services

11 6. Surveillance, Evaluation and Research Management
Support the protection of community data Standardize data collection and reporting including tools to support FNIHB collating, analyzing, and sharing regional data, and assisting communities upon request

The ideas shared today can be added to the ever- changing HIV/STBBI Action Plan The action plan can be used in part or whole to guide program decisions and services at the community, tribal, intertribal and regional levels As the Steering Committee further explores evolving to an advisory committee of health expertise, this plan will include more detail and timelines
